Example Credit Card Payment


Policy Information:
Type in your policy number or note number exactly as it appears on your Policy DEC page or Payment Coupon.

Customer Information:
Please fill out this section completely.

If you enter a valid email address an email confirmation will be sent to that address shortly after authorizing the payment.

Credit Card Information:
Enter all information from your Credit Card. (VISA, MASTERCARD, DISCOVER ONLY)

Please enter the name as it appears on the CC, select your card type and enter the 16 digit card number without any spaces or dashes.

Select the month and year the card expires and enter the security code from the back of the card as the graphic shows.

Enter the payment amount in this format 000.00

After clicking the "Authorize" button a page similar to this will be displayed. Click "Print" to print the page if your computer has a printer connected to it.

The "Confirmation number" can be used to reference this submission if needed.

Example Check Payment

Policy Information:
Type in your policy number or note number exactly as it appears on your Policy DEC page or Payment Coupon.
Customer Information:
Please fill out this section completely. If you enter a valid email address an email confirmation will be sent to that address shortly after authorizing the payment.

Bank Account/Check Information:
Enter all information requested. Please make sure the Name on Acct. is as it appears on the bank account.

You must enter your account number twice.

Enter your check number 3 to 5 digits.

Enter the payment amount in this format 000.00

You can setup the Auto-Pay feature by
checking the "Yes, Auto-Pay" box. This will enable the "# of Payments" selector. Select the number of payments including the payment being made now.

For example...
If you owe 12 payments and you are making the first payment, you would select 12 as the "# of payments".
